    Monticello Youth Baseball Registration 2022

    Must be 4 by April 30th
    Monticello Youth Baseball will be holding All-Star try-outs separate from normal baseball season.  Anyone interested will be required to try out.  Playing for Monticello Youth Baseball Travel Team does not automatically give you a spot on the All-Star Team.  Games normally start the first week in July and can possibly continue through July into August.
    1.  I/We, the parents/guardians of the above named child applying for a position on a Monticello Youth Baseball team, hereby give my/our approval to participate in any and all Youth Baseball activities.
    2.  I/We know that participation in baseball may result in serious injuries and protective equipment does not prevent all injuries to players and do hereby waive, release, absolve, indemnify, and agree to hold harmless the Monticello Youth Baseball league, the organizers, sponsors, supervisors, and participants from any claim airing out of any injury to my/our child whether the result of negligence or for any other cause.
    3.  I/We agree to provide proof of age in the form of a Birth Certificate

    COVID-19 Waiver

     WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19 for 2021 season
    ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT 
    In consideration of being allowed to participate on behalf of Twin Lakes School Corporation athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
    1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
    2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation; and,
    3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
    4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless any of the above named entities, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASES”), with respect to any and all illness, disability, death, or loss or damage to persons or property, whether arising from the negligence of releases or otherwise, to the fullest extent permitted by law. 
    5.  I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation when choosing not to wear a face covering. Do not come to practice or games if you have a fever. Please be diligent on checking temperatures every day!
     
     
    I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY
    UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.  
     
    FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
    This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releases and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releases for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE to the fullest extent provided by law.

    Medical Release

    As parents/guardians of the above mentioned player, I hereby give my consent for emergency medical care prescribed by a licensed physician or dentist.  This care may be given under whatever conditions are necessary to preserve life, limb, or well being of my dependent.  You have my permission to take whatever action is deemed necessary for the health and welfare of my child.

    Parent code of conduct

    I hereby pledge to provide positive support, care, and encouragement for my child participating in youth sports by following this Parents’ Code of Conduct. 
    I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game.
    I will place the emotional and physical well-being of my child ahead of my personal desire to win. 
    I will insist that my child play in a safe and healthy environment.   
    I will support coaches and officials working with my child, in order to encourage a positive and enjoyable experience for all. 
    I will demand a sports environment for my child that is free from drugs, tobacco and alcohol and will refrain from their use at all youth sports events. 
    I will remember that the game is for youth - not adults. 
    I will do my very best to make youth sports fun for my child. 
    I will ask my child to treat other players, coaches, fans and officials with respect regardless of race, sex, creed or ability. 
    I will help my child enjoy the youth sports experience by doing whatever I can, such as being a respectful fan, assisting with coaching, or providing transportation. 
    I promise to make a commitment to volunteer and assist the Monticello Youth Baseball system when asked, making time whenever I can.   
    I understand that if at any time my conduct is demeaning or improper at any game or practice, I will be asked to leave the sports facility and be suspended from the following game. Repeat violations may cause a multiple game suspension, or the season forfeiture of the privilege of attending any game.

    Players' code of conduct

    I hereby pledge to be positive about my youth sports experience and accept responsibility for my participation by following this Players' Code of Ethics Pledge.

    Have fun!

    Be a good sport (win or lose); Be honest, fair, and always show good sportsmanship to all coaches, players, officials, parents, and fans by demonstrating good sportsmanship at every game and practice.  Learn the value of commitment to the team by attending every practice and game that you can.  Notify the coach if you can't.  Put personal goals aside for the betterment of the team.  Show courtesy and respect to teammates, opponents, and coaches.  Realize that athletic contests, including practice sessions are educational experiences and opportunities.  

    Pledge the following:
    I will not engage in unsportsmanlike conduct.
    I will not engage in rude behavior.
    I will treat everyone, including coaches, parents, players, and officials, with respect, regardless of race, creed, nationality, or gender.

    Any player who is disciplined for unsportsmanlike behavior will not be eligible for All-Stars.

    First Offense:  The player will be given a verbal warning.
    Second Offense:  The player will be suspended for the next game following the offense.
    Third Offense:  The player will be removed from the league for the remainder of the season.
